Tooth 16 Is Not a Finding. The Surface Is.

A note that says "tooth 16, caries" proves a problem exists. It cannot tell you where the lesion sits, how many surfaces it crosses, or what to charge for fixing it.

What is per surface dental charting?

Per surface dental charting records findings where they actually live: on the mesial, distal, occlusal, buccal, or lingual surface of a specific tooth. "MO caries on 16" and "small occlusal pit on 16" are different diagnoses, procedures, chair times, and prices. A chart that only stores "caries, 16" flattens all four into one word.

Most paper charts in Philippine clinics were never designed for that detail. There is a diagram, a pencil shade, and a hope that whoever reads it next remembers what the shading meant. When the associate who charted it resigns, the meaning resigns with them.

What do tooth-level notes cost you downstream?

The damage never shows during the exam. It shows after:

  • Plans stay vague. "Caries, 16" cannot say whether the patient needs a one surface restoration or a three surface buildup. Vague plans get postponed, and postponed plans quietly die, which is why treatment plans that patients accept start at the surface level.
  • Prices get guessed. Clinics price restorations per surface, but if the chart does not count surfaces, the front desk guesses. Undercharging eats margin, and overcharging eats trust.
  • Continuity breaks. A patient returns after two years, and nobody can say whether the distal lesion on 24 is the one you restored or a new one. You re-diagnose from scratch.
  • Disputes drag. "Class II restoration, mesio-occlusal, tooth 16, charted on this date" ends an HMO question. "Filling" starts one.

One entry at the chair, three jobs done

In a properly built system, the surface is the unit of record. The dentist taps tooth 16, taps mesial and occlusal, selects the finding, and that single entry does three jobs: it renders on the visual chart for the next clinician, it feeds the treatment plan as a specific procedure with a specific fee, and once completed it becomes the invoice line, priced by surface count, with zero retyping.

That is the whole trick. Not more typing at the chair, but one entry captured where the finding happened, reused three times. The same completed line lands in your daily close numbers that evening, and the paperwork has assembled itself.

How much does guessing surfaces cost?

Run an example. If a one surface composite is 1,500 pesos, each added surface is 500, and the guess runs low by one surface on just three restorations a day, that is 1,500 pesos a day of quietly forgiven work. Over a six day week, around 9,000 pesos; over a year, several hundred thousand.

Those are illustrative numbers, not a promise. Plug in your own fees, because the point survives any figures: when the record is coarser than the price list, the price list loses. The leak never shows on a report either, because the invoice always matches the guess.

Four questions that expose a shallow chart

Skip the demo gloss and ask: can it record a finding on a surface rather than a tooth, turn that finding into a plan item without retyping, turn the completed item into an invoice line, and show one tooth's surface history across five years of visits?
